Summary

For bicuspid aortic valve patients needing surgery, aortic root replacement offers similar survival outcomes to isolated aortic valve replacement. This suggests a more aggressive surgical approach may be beneficial for preventing future aortic complications.

Background:

  • Bicuspid aortic valve (BAV) patients have an increased risk of ascending aortic aneurysm due to aortic wall abnormalities.
  • A lower threshold for aortic surgery may be considered in BAV patients, particularly those requiring aortic valve surgery.

Purpose of the Study:

  • To compare outcomes of isolated aortic valve replacement (AVR) versus aortic root replacement in BAV patients undergoing aortic valve surgery.

Main Methods:

  • Retrospective study of elective AVR or composite valve-graft conduit procedures (2006-2021) in BAV patients.
  • National database used for mortality data retrieval; survival analyses performed for comparison between groups.

Main Results:

  • 132 isolated AVR and 149 aortic root replacements were analyzed.
  • Patients undergoing isolated AVR were older (62.9 vs. 57.7 years).
  • Long-term mortality was comparable between isolated AVR and aortic root replacement groups (8.1% vs. 9.1%).

  • A more aggressive surgical approach for BAV aortopathy may be considered to prevent future aortic complications in this high-risk group.
