This case report details managing a neglected talonavicular joint (TNJ) dislocation in a six-month-old. Corrective osteotomy and subtalar fixation with joint fusion successfully relieved pain.
Area of Science:
- Orthopedics and Traumatology
- Podiatric Surgery
- Medical Case Reports
Background:
- Talonavicular joint (TNJ) dislocation involves complete loss of articulation between the talus and navicular bones.
- Medial TNJ dislocations are more common; lateral dislocations often accompany calcaneocuboid fractures.
- Treatment challenges exist, with high-energy trauma necessitating reduction and immobilization, sometimes requiring open reduction and fixation.
Observation:
- A six-month-old patient presented with a neglected talonavicular joint dislocation.
- The case involved complex joint disruption requiring surgical intervention.
- Standard immediate reduction and immobilization were not applicable due to the neglected nature of the dislocation.
Findings:
- A corrective osteotomy and subtalar fixation were performed for the neglected TNJ dislocation.
- Fusion of the talonavicular and talocalcaneal joints was achieved during the procedure.
- The surgical management resulted in a satisfactory outcome for the patient.
Implications:
- This case demonstrates the efficacy of corrective osteotomy and subtalar fixation in managing neglected TNJ dislocations.
- Surgical fusion of adjacent joints can restore alignment, strength, and function in complex foot and ankle trauma.
- Complete pain relief was achieved, highlighting the potential for successful functional recovery even in delayed presentations.

Fractures: Bone Repair
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the procedure...
