Background:

  • Clinician-ordered peripheral smear review (CPSR) is a long-standing diagnostic practice.
  • The clinical utility of CPSR in reference laboratories remains largely unexamined.
  • This study investigates the value of CPSR compared to automated methods.

Purpose of the Study:

  • To assess hematologic abnormalities detected by CPSR.
  • To correlate CPSR findings with complete blood count (CBC) and laboratory-derived smear review (LDSR).
  • To evaluate the necessity of CPSR in a reference laboratory setting.

Main Methods:

  • Reviewed 200 consecutive CPSRs and corresponding CBCs.
  • Analyzed peripheral smears using Sysmex® XN-11 analyzers.
  • Hematopathologist assessed abnormalities and correlated results with LDSR.

Main Results:

  • 29% of CPSRs showed normal smears and CBCs.
  • Most CPSRs revealed nonspecific quantitative abnormalities.
  • LDSR correctly identified 100% of hematologic abnormalities, with one minor discrepancy.

Conclusions:

  • Laboratory-derived smear review (LDSR) adequately detects significant hematologic abnormalities.
  • CPSR is not essential in the reference laboratory setting.
  • Discontinuation of CPSR is recommended to optimize laboratory resources.
