Area of Science:

  • Otolaryngology
  • Surgical Innovation
  • Audiology

Background:

  • Myringoplasty techniques have evolved since 1952, with overlay and underlay methods being common.
  • The cartilage and perichondrium butterfly inlay myringoplasty, introduced in 1998, is designed for small to medium perforations.

Purpose of the Study:

  • To evaluate the efficacy of cartilage rosette inlay-onlay myringoplasty for small central perforations.
  • To assess graft uptake rates and hearing outcomes post-surgery.

Main Methods:

  • A prospective study of 30 patients (aged 9-52) with inactive small central perforations.
  • Pre-operative pure tone audiometry (PTA) up to 35 dB conductive hearing loss.
  • Transcanal insertion of cartilage graft under general or local anesthesia.

Main Results:

  • 100% graft uptake at 3 months and 93.33% at 6 months.
  • Mean PTA improved from 29.76 dB pre-operatively to 25.03 dB post-operatively.
  • Statistically significant air-bone gap closure of 4.73 dB.

Conclusions:

  • Cartilage rosette inlay-onlay myringoplasty is effective for small to medium perforations.
  • The technique offers high success rates, reduced surgical time, and suitability for day care surgery.
  • Minimal scarring and improved hearing make it valuable for clinical practice.
