Summary

Transgender adults prefer long-acting injectable HIV pre-exposure prophylaxis (LA-PrEP) when integrated with gender-affirming care, including hormone therapy. This integration is key for uptake of HIV prevention methods among this population.

Background:

  • Integrating HIV prevention with gender-affirming care is a promising strategy to address this disparity.

Purpose of the Study:

  • To identify key programming factors influencing trans adults' decisions to use injectable long-acting HIV pre-exposure prophylaxis (LA-PrEP).
  • To inform the development of effective LA-PrEP programs tailored to the needs of trans adults.

Main Methods:

  • A discrete choice experiment was conducted with 366 trans adults in Washington state.
  • Participants evaluated hypothetical LA-PrEP programs based on various attributes.
  • Data were analyzed using a mixed rank-ordered logit model.

Main Results:

  • Respondents strongly preferred receiving LA-PrEP from a gender-affirming care provider alongside hormone therapy.
  • A preference for 12-month protection and upper arm injections was observed.
  • No significant preference for specific health facility types was identified.

Conclusions:

  • Integrating LA-PrEP within gender-affirming health systems, including hormone therapy, is highly preferred by trans adults.
  • Choice-based design experiments are valuable for optimizing LA-PrEP programming for this population.
