    Summary

The automatic cardioverter-defibrillator (AICD) effectively treats life-threatening ventricular arrhythmias. This advanced device significantly increases survival rates in high-risk patients with sudden arrhythmic death syndrome.

    Area of Science:

    • Cardiology
    • Biomedical Engineering

    Background:

    • Over 400 survivors of sudden arrhythmic death have been treated with automatic implantable defibrillators since 1980.
    • The automatic cardioverter-defibrillator (AICD) represents an advancement, capable of treating both ventricular fibrillation and ventricular tachycardia.

    Purpose of the Study:

    • To evaluate the efficacy of the AICD in managing potentially lethal ventricular tachyarrhythmias.
    • To assess the impact of AICD implantation on patient survival rates.

    Main Methods:

    • Implantation via thoracotomy, subxiphoid, or subcostal approach.
    • Utilized dual defibrillating electrodes for waveform analysis and a third bipolar electrode for rate counting and R-wave synchronization.
    • Delivered 25-joule pulses for ventricular fibrillation and R-wave synchronized discharges for ventricular tachycardia.

    Related Experiment Videos

    Main Results:

    • The AICD demonstrated reliable identification and correction of ventricular tachyarrhythmias.
    • Actuarial analysis showed a reduction in one-year mortality attributed to arrhythmias to less than 2% over a 58-month follow-up.
    • Device batteries support over 100 shocks or three years of monitoring.

    Conclusions:

    • The AICD is a reliable tool for managing life-threatening ventricular arrhythmias.
    • AICD implantation leads to a substantial increase in survival for carefully selected high-risk patients.
