Summary

Nasal obstruction surgery effectively improves symptoms for patients with rhinitis medicamentosa (RM) and those without. Most RM patients successfully cease topical nasal decongestant use after surgery, maintaining long-term relief.

Background:

  • Topical nasal decongestants (TNDs) alleviate nasal obstruction but can lead to rhinitis medicamentosa (RM), characterized by rebound edema.
  • Current RM management primarily involves TND cessation, often with intranasal corticosteroids.

Purpose of the Study:

  • To compare nasal obstruction outcomes after surgery in patients with and without RM.
  • To evaluate the efficacy of nasal obstruction surgery in RM patients.

Main Methods:

  • Retrospective case-control study of adult patients undergoing bilateral inferior turbinate reduction (ITR) with or without other nasal surgeries.
  • RM defined as daily TND use for ≥4 weeks.
  • Nasal Obstruction Symptom Evaluation (NOSE) scores and TND cessation rates were analyzed.

Main Results:

  • Both RM and non-RM groups showed significant postoperative NOSE score reductions (p < 0.0001).
  • No significant difference in NOSE score reduction was observed between the RM and non-RM groups (p = 0.2438).
  • 86.1% of RM patients maintained long-term TND cessation post-surgery.

Conclusions:

  • Nasal obstruction surgery provides comparable long-term symptom relief for patients with and without RM.
  • Surgery is an effective option for RM patients, facilitating TND cessation and sustained improvement.
