Myxofibrosarcoma (MFS), a soft-tissue sarcoma, presents unique challenges. This case highlights the diagnosis and integrated management of a large, aggressive MFS in an elderly patient.

- A large (25 x 20 cm²) spindle cell sarcoma with ulceration was observed on the anterior right thigh of a woman in her 50s.

- Histopathological analysis confirmed a grade 2 Myxofibrosarcoma.
