Loop ileostomy for anorectal Crohn's disease.

    Summary

    Fecal diversion using a loop ileostomy can temporarily relieve anorectal Crohn's disease symptoms. However, it does not alter the disease's long-term progression, and restoring intestinal continuity is rarely successful.

    Purpose of the Study:

    • To evaluate the clinical outcomes of fecal diversion via loop ileostomy in patients with anorectal Crohn's disease.

    Main Methods:

    • Retrospective review of 12 patients with symptomatic anorectal Crohn's disease treated with loop ileostomy.
    • Analysis of outcomes including symptom resolution, fistula healing, and restoration of intestinal continuity.

    Main Results:

    • Loop ileostomy provided temporary symptom improvement in most patients.
    • Seven patients with rectovaginal fistulas had varied outcomes after ileostomy closure.
    • Five patients with proctitis or sepsis also showed mixed results post-ileostomy.
    • Restoration of intestinal continuity was uncommon, and proctocolectomy was required in several cases.

    Conclusions:

    • Loop ileostomy offers temporary symptomatic relief for anorectal Crohn's disease.
    • Fecal diversion does not significantly impact the long-term disease course.
    • Successful reversal of ileostomy and restoration of bowel continuity is infrequent.
