Insights

This study describes the first documented ultrasound findings in rare post-streptococcal myalgia and myositis. Muscular ultrasound aids in diagnosing this condition following streptococcal infections.

Observation:

  • A 31-year-old female presented with severe thigh myalgia after a streptococcal upper respiratory infection.
  • Erythema nodosum and elevated muscle enzymes (Creatine Kinase, Lactate Dehydrogenase) and Antistreptolysin antibodies confirmed post-streptococcal myositis.

Findings:

  • Ultrasound revealed focal muscle echogenicity increase, architectural distortion, and hypervascularity in the affected vastus medialis muscle.
  • Magnetic Resonance Imaging confirmed focal myositis.
  • Symptoms and ultrasound findings resolved with Ibuprofen and Ampicillin/Sulbactam treatment.

Implications:

  • This case presents the initial description of ultrasound findings in post-streptococcal myositis.
  • Muscular ultrasound is a valuable tool for diagnosing myositis in the context of post-streptococcal syndromes.
  • Early diagnosis and treatment can lead to full recovery.
