    Adrenal hemangiomas are rare tumors, with only 18 cases documented globally. Computed tomography scans often fail to distinguish these tumors from other adrenal masses unless specific calcifications are present.

    Background:

    • Adrenal hemangiomas are exceptionally rare benign vascular tumors originating from the adrenal gland.
    • Diagnosis is challenging due to their infrequent occurrence and nonspecific imaging characteristics.

    Observation:

    • This report details the 18th surgically or autopsy-proven case of adrenal hemangioma in the world literature.
    • The tumor was an incidental finding during an autopsy examination.

    Findings:

    • Computed tomography (CT) findings for adrenal hemangioma are nonspecific.
    • Differentiation from other primary or secondary adrenal tumors via CT is not possible without characteristic calcifications or phleboliths.

    Implications:

    • The nonspecific CT appearance necessitates a high index of suspicion for adrenal hemangioma in rare cases.
    • Further investigation may be required to differentiate adrenal hemangioma from malignant adrenal neoplasms when calcifications are absent.
