The platelet phase, the second stage of hemostasis, commences around 15-20 seconds after an injury. It follows and overlaps with the vascular phase, during which blood vessels constrict to minimize blood loss.

Summary

Platelet distribution width (PDW) shows a causal link to poor functional outcomes after ischemic stroke. While higher PDW levels correlate with worse outcomes, its predictive value may be indirect, possibly related to glucose levels.

Background:

  • Complete blood cell count (CBC) is a common lab test.
  • Observational studies suggest links between CBC indices and ischemic stroke, but causality is unclear.

Purpose of the Study:

  • To investigate the causal relationships between CBC indices and functional outcomes (modified Rankin Scale score) 3 months post-ischemic stroke.
  • To explore the role of platelet distribution width (PDW) in stroke prognosis.

Main Methods:

  • Two-sample Mendelian randomization (TSMR) using UK Biobank data for blood traits (n=350,475) and GISCOME network data for ischemic stroke outcomes (n=6021).
  • Complementary analyses and a clinical cohort validation (n=351) were performed.

Main Results:

  • TSMR revealed a causal association between genetically determined PDW and adverse functional outcomes after ischemic stroke (OR 1.48, P=0.005).
  • Higher PDW levels were observed in patients with unfavorable outcomes in the cohort study.
  • PDW showed positive correlation with serum glucose and was higher in non-thrombolysed patients, suggesting indirect involvement in prognosis.

Conclusions:

  • Genetically determined PDW has a causal effect on poor functional outcomes following ischemic stroke.
  • PDW's role in stroke prognosis appears indirect, potentially mediated by factors like serum glucose levels.
