Patients with brain arteriovenous malformations and aneurysms require careful management. Treating aneurysms before arteriovenous malformation resection is recommended to prevent hemorrhage risk.

- 22 patients with intracranial aneurysms and AVMs were evaluated.

- 78% of hemorrhages originated from aneurysms, primarily atypical distal ones.
- 7 patients with aneurysmal hemorrhage all bled from distal aneurysms on major feeding vessels.
- Aneurysmal hemorrhage occurred in 78% of patients presenting with intracranial hemorrhage.
Implications:
- Treating the aneurysm prior to AVM resection is the safest approach.
- Sudden AVM elimination can pose immediate risks to associated aneurysms.
- This approach mitigates risks associated with hemodynamic shifts post-AVM treatment.
