Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

369
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Area of Science:

  • Biomedical Engineering
  • Fluid Dynamics
  • Laser Physics

Background:

  • Traditional needle-based injections pose risks like discomfort, infection, and patient non-compliance.
  • Needle-free injection systems are sought to mitigate these issues in medical treatments.

Purpose of the Study:

  • To investigate bubble laser technology as a viable needle-free alternative for drug delivery systems.
  • To analyze the performance and parameters influencing laser-induced bubble dynamics for injection applications.

Main Methods:

  • Analysis of laser parameters (wavelength, pulse duration) and their effect on bubble dynamics and jet formation.
  • Utilizing the Rayleigh-Plesset equation to model drug delivery characteristics.
  • Experimental investigation of bubble generation, jetting, and fluid interactions.

Main Results:

  • Spot size increased significantly at 1064 nm compared to 400 nm, aligning with diffraction theory.
  • Bubble dynamics, jet velocity, and fluid inertia were observed to increase with the Weber number.
  • Shorter laser pulses in water enhanced jet formation and bubble longevity, crucial for effective drug delivery.

Conclusions:

  • Bubble laser technology provides a controlled and safer needle-free injection method.
  • Optimizing laser parameters like pulse duration influences bubble lifespan and collapse pressure, impacting drug delivery efficiency.
  • This technology has the potential to increase patient compliance and reduce tissue trauma in medical procedures.
