    Summary

    Intracarotid chemotherapy using 1,3-bis(2-chloroethyl)-1-nitrosurea (BCNU) can cause severe leukoencephalopathy. This toxicity, characterized by brain necrosis and edema, was observed in a patient with high-grade astrocytoma.

    Background:

    • High-grade glial neoplasms are treated with intracarotid 1,3-bis(2-chloroethyl)-1-nitrosurea (BCNU).
    • The toxicity profile of intracarotid BCNU therapy is not well-documented.
    • This study investigates a case of severe neurotoxicity following BCNU treatment.

    Observation:

    • A 50-year-old man with grade 4 astrocytoma received three intracarotid BCNU injections.
    • Initial tumor bulk reduction was observed, but the patient's condition deteriorated after the third dose, leading to death 5 weeks later.
    • Autopsy revealed extensive brain cavitation, necrosis, vascular damage, and edema confined to the BCNU perfusion territory.

    Findings:

    • Intracarotid BCNU administration led to severe leukoencephalopathy.
    • Pathological findings included coagulative necrosis, fibrinoid vascular necrosis, edema, and axonal swelling.
    • Tumor persisted in areas outside the BCNU perfusion zone.

    Implications:

    • Intracarotid BCNU can induce severe leukoencephalopathy, mimicking methotrexate toxicity or delayed radiation necrosis.
    • This finding highlights the critical need for careful monitoring and risk-benefit assessment in neuro-oncology treatments.
    • Further research is warranted to understand and mitigate BCNU-induced neurotoxicity.
