Summary

Ceramic and metal humeral hemiarthroplasties both caused significant glenoid cartilage wear in vitro. Neither material demonstrated superior performance, suggesting ceramic may not be a better alternative for shoulder hemiarthroplasty in patients with healthy cartilage.

Background:

  • Humeral hemiarthroplasty is used for shoulder joint restoration.
  • Glenoid erosion is a complication that can limit implant survival.
  • Ceramic materials offer potential advantages over metal due to superior tribological properties.

Purpose of the Study:

  • To compare the in vitro wear performance of ceramic versus metal humeral hemiarthroplasties.
  • To assess the impact of these materials on natural glenoid cartilage erosion.

Main Methods:

  • Utilized a shoulder wear simulator with cadaveric shoulders (ceramic n=8, metal n=9).
  • Simulated daily activity for 500,000 cycles with lubrication.
  • Quantified glenoid cartilage wear using micro-CT scans at 125,000-cycle intervals.

Main Results:

  • Both ceramic and metal hemiarthroplasties led to significant glenoid cartilage thickness reduction (27% and 29%, respectively).
  • No significant difference in wear was observed between ceramic and metal groups (p=0.606).
  • Wear patterns were unpredictable, with no correlation to patient or implant factors.

Conclusions:

  • Despite ceramic's favorable tribological properties, it did not show superior wear performance compared to metal in this study.
  • Current evidence does not support ceramic hemiarthroplasty as a better alternative to metal for shoulders with intact glenoid cartilage.
