Summary

Patients undergoing transplanum and transtuberculum expanded endonasal approaches may experience prolonged olfactory loss. Narrower angles between the planum and sella face, and anterior nasal spine and sphenoid sinus, are identified as risk factors.

Background:

  • Expanded endonasal approaches (EEA) are increasingly used for skull base pathologies.
  • Olfactory dysfunction is a potential complication of EEA.
  • Identifying risk factors for olfactory dysfunction is crucial for patient counseling and surgical planning.

Purpose of the Study:

  • To determine clinical characteristics and cephalometric risk factors for decreased postoperative olfaction after transplanum and transtuberculum EEA.

Main Methods:

  • Retrospective cohort study of 41 patients undergoing transplanum and transtuberculum EEA.
  • Patients were divided into prolonged olfactory loss (n=5) and preserved olfaction (n=36) groups based on SNOT22 olfaction scores.
  • Demographics, surgical details, and cephalometric measurements were compared.

Main Results:

  • No significant differences in surgical approach, turbinate resection, flap reconstruction, or tumor pathology.
  • Prolonged olfactory loss group showed a smaller angle between the planum and sella face (89.75° vs 107.17°, p=0.05).
  • Prolonged olfactory loss group had a smaller angle between the anterior nasal spine and sphenoid sinus face (21.20° vs 25.89°, p=0.047).

Conclusions:

  • Cephalometric measurements, specifically narrower angles between the planum-sella and anterior nasal spine-sphenoid sinus, are associated with prolonged olfactory dysfunction after transplanum and transtuberculum EEA.
  • These anatomical variations may predispose patients to olfactory deficits following these specific surgical approaches.
