Summary

Traumatic chronic subdural hematoma (CSDH) patients are younger and more often asymptomatic or have seizures. Not otherwise specified (NOS) CSDH is more common in older individuals taking blood thinners, with similar outcomes for both groups.

Background:

  • Chronic subdural hematoma (CSDH) is a prevalent neurological condition with rising incidence.
  • CSDH can occur with or without a history of head trauma.
  • Understanding clinical differences between traumatic and non-traumatic CSDH is crucial for patient management.

Purpose of the Study:

  • To investigate the clinical distinctions between traumatic and not otherwise specified (NOS) chronic subdural hematoma (CSDH).
  • To compare baseline characteristics, radiological findings, treatment, and outcomes of traumatic versus NOS CSDH.

Main Methods:

  • Retrospective cohort study including 168 traumatic CSDH and 133 NOS CSDH patients.
  • Data collected included demographics, clinical features, anticoagulant/antiplatelet use, radiological findings, treatment, and outcomes.
  • Statistical analysis compared parameters between the two groups.

  • Traumatic CSDH patients were younger, had higher rates of epilepsy and asymptomatic presentation, and were less likely to use anticoagulants/antiplatelets compared to NOS CSDH (P < 0.05).

  • Treatment and clinical outcomes for traumatic and NOS CSDH are comparable.
