Robot-assisted partial nephrectomy (RAPN) offers a feasible solution for multifocal allograft renal cell carcinoma (RCC). This nephron-sparing approach successfully preserved kidney function in a transplant recipient with multiple tumors.

  • Partial nephrectomy (PN) is the standard for T1 renal tumors.
  • Robot-assisted PN (RAPN) is reported for solitary allograft tumors, but rarely for multifocal disease.
  • This case explores RAPN for multifocal allograft renal cell carcinoma (RCC).

Observation:

  • A 77-year-old male presented with two renal allograft tumors (24mm and 15mm).
  • RAPN was performed via transperitoneal approach 22 years post-transplant.
  • Tumor resection achieved negative surgical margins for clear-cell RCC.

Findings:

  • Robot-assisted partial nephrectomy (RAPN) proved feasible and effective for multifocal allograft RCC.
  • The procedure resulted in no significant change in kidney function.
