Background:

  • Major depressive disorder (MDD) is associated with cardiovascular risk factors and cerebral small-vessel disease.
  • Endothelial dysfunction is linked to MDD, but prospective data on peripheral endothelial dysfunction (PED) and incident MDD is lacking.

Purpose of the Study:

  • To investigate the prospective association between peripheral endothelial dysfunction (PED) and the incidence of major depressive disorder (MDD).

Main Methods:

  • Evaluated 1614 patients for PED using reactive hyperemia-peripheral arterial tonometry.
  • Reviewed medical records to identify 484 patients with subsequent MDD evaluations (0-15 years post-PED assessment).
  • Used logistic regression to analyze the association between baseline PED and incident MDD.

Main Results:

  • 30.1% of patients had PED; 31.0% were diagnosed with MDD.
  • Individuals with MDD showed a higher frequency of PED (40.2% vs. 30.2%, P=0.034).
  • PED was significantly associated with incident MDD (OR, 2.3; 95% CI, 1.4-3.8; P<0.001).

Conclusions:

  • Peripheral endothelial dysfunction is significantly associated with incident major depressive disorder.
  • PED may serve as a valuable biomarker for identifying individuals at elevated risk for depression.
  • Early and frequent management strategies may benefit individuals identified with PED.
