This international survey reveals a wide variation in fecal immunochemical test (FIT) positivity thresholds used globally for colorectal cancer screening. Thresholds are adjusted to balance sensitivity and available colonoscopy resources, highlighting the need for flexible test endpoints in new screening technologies.
