Tracheostomy Decannulation01:21

Tracheostomy Decannulation

115
Tracheostomy decannulation is a significant milestone in the liberation of mechanically ventilated patients. Despite its importance, there is no universally accepted protocol for this procedure. This demands an evidence-based, individualized approach.
Description of the Procedure
Decannulation refers to the permanent removal of the tracheostomy tube, signaling the resolution of the condition that initially necessitated the tracheostomy. The process requires a well-coordinated interplay between...

Cancer Survival Analysis01:21

Cancer Survival Analysis

328
Cancer survival analysis focuses on quantifying and interpreting the time from a key starting point, such as diagnosis or the initiation of treatment, to a specific endpoint, such as remission or death. This analysis provides critical insights into treatment effectiveness and factors that influence patient outcomes, helping to shape clinical decisions and guide prognostic evaluations. A cornerstone of oncology research, survival analysis tackles the challenges of skewed, non-normally...

Area of Science:

  • Otolaryngology
  • Surgical Oncology
  • Health Disparities Research

Background:

  • Existing research highlights racial disparities in laryngeal cancer treatment and survival.
  • However, immediate postoperative outcomes following total laryngectomy (TL) lack comprehensive investigation regarding racial differences.

Purpose of the Study:

  • To investigate racial and ethnic disparities in adverse outcomes within 30 days following total laryngectomy (TL).

Main Methods:

  • A national multi-institutional cohort of 1493 patients undergoing TL between 2010 and 2021 was analyzed.
  • Logistic regression models were used to assess the association between race/ethnicity and postoperative complications.

Main Results:

  • Black patients were more likely to undergo flap reconstruction compared to other racial groups (P=.023).
  • Black patients demonstrated a 1.532 times higher odds of experiencing surgical complications within 30 days post-TL (P<.001).

Conclusions:

  • Black patients undergoing total laryngectomy experience significantly higher rates of surgical complications.
  • Future research should explore socioeconomic and oncologic factors contributing to these disparities, given data limitations.
