Background:

  • Multilayer compression bandaging (MLB) is a primary lymphedema treatment.
  • MLB is contraindicated in patients with advanced lower extremity artery disease (LEAD).
  • The prevalence of LEAD in lymphedema patients is currently unknown.

Purpose of the Study:

  • To estimate the prevalence of LEAD (defined by toe-brachial index [TBI] < 0.7) in lymphedema patients.
  • To measure the evolution of TBI after 30 minutes of MLB.

Main Methods:

  • A 3-month cross-sectional study.
  • Included patients with lower extremity lymphedema.
  • Recorded demographic data, basal TBI (T=0min), and TBI after 30 min of MLB (T=30min).

Main Results:

  • 27.6% of lymphedema limbs (n=8/29) had non-symptomatic LEAD (TBI < 0.7).
  • LEAD was associated with advanced age, severe stages, and longer lymphedema duration.
  • Median TBI significantly increased after MLB (0.81 to 0.96, p=0.004).

Conclusions:

  • Subclinical LEAD is prevalent in over a quarter of lymphedema limbs.
  • LEAD is underestimated in lymphatic pathologies.
  • MLB may improve TBI, but careful patient selection is crucial.
