    Summary

    Kingella denitrificans rarely causes endocarditis, especially in prosthetic valve cases. Successful treatment with antibiotics like penicillin G and ampicillin was achieved without surgery.

    Background:

    • Kingella species are infrequently documented as causative agents of endocarditis.
    • Prosthetic valve endocarditis (PVE) presents a significant clinical challenge.

    Observation:

    • Two cases of Kingella denitrificans endocarditis in patients with prosthetic valves are presented.
    • This represents the first reported instance of K. denitrificans causing prosthetic valve endocarditis.

    Findings:

    • Bacteremia likely originated from the upper respiratory or oral mucosa, leading to prosthetic valve infection.
    • Patient 1 achieved a clinical cure with penicillin G and streptomycin.
    • Patient 2 experienced a clinical cure with ampicillin monotherapy.
    • In vitro antibiotic susceptibility revealed high sensitivity to cefotaxime and ampicillin for both isolates.

    Implications:

    • Kingella denitrificans should be considered in the differential diagnosis of PVE.
    • Antibiotic therapy, particularly with penicillin or ampicillin, can be effective in treating K. denitrificans endocarditis.
    • Further research into the pathogenesis and optimal treatment strategies for Kingella-related endocarditis is warranted.
