    Summary

    A modified Amsler grid test, using low luminance conditions, significantly improves the detection of visual field defects in optic neuropathies. This enhanced method is a rapid, inexpensive, and sensitive tool for evaluating central vision.

    Background:

    • The standard Amsler grid is a suprathreshold test, potentially missing relative scotomas in visual field evaluation.
    • Optic neuropathies can present with subtle visual field deficits not always detected by conventional methods.

    Purpose of the Study:

    • To investigate the efficacy of a threshold Amsler grid technique under low luminance conditions for detecting visual field defects.
    • To compare the diagnostic yield of the modified Amsler grid with standard visual field testing methods.

    Main Methods:

    • Ten patients with optic neuropathies underwent testing with a cross-polarizing filter-modified Amsler grid at low luminance.
    • Patients were also evaluated using a 2-m tangent screen and threshold automated perimetry.
