The Dermatology-Life-Quality-Index (DLQI) in Colombia requires refinement. Rasch analysis indicated the current DLQI-COL is not suitable for evaluating quality of life in Colombian dermatology patients.
Area of Science:
- Dermatology
- Psychometrics
- Health Outcomes Research
Background:
- Previous validation of the Dermatology-Life-Quality-Index (DLQI) in Colombia using classical test theory highlighted the need for further psychometric evaluation.
- Item response theory (IRT) offers advanced methods to assess measurement properties beyond traditional approaches.
Purpose of the Study:
- To assess the structural validity, internal consistency, and item response characteristics of the DLQI in a Colombian population using the Rasch model.
- To evaluate the DLQI's performance in differentiating between inflammatory and non-inflammatory skin diseases.
Main Methods:
- Rasch model analysis was applied to data from 865 Colombian patients with various skin diseases.
- Dimensionality, item difficulty, discrimination, and differential item functioning were assessed.
- Internal consistency and discriminative validity were evaluated.
Main Results:
- Horn's parallel analysis supported the unidimensionality of the DLQI with a one-factor solution.
- Internal consistency measures (McDonald's Omega and Cronbach's alpha) were generally high for item #7.
- Most items, except item #1, did not fit the Rasch partial credit model, indicating potential issues with response patterns.
Conclusions:
- The DLQI, in its current Colombian adaptation (DLQI-COL), is not a suitable instrument for measuring quality of life in this population based on Rasch analysis.
- A revised version of the DLQI, with improved cross-cultural adaptation and validation, is necessary for reliable quality of life assessment in Colombian dermatology patients.
