Suxamethonium chloride and malignant hyperpyrexia.

G J Galloway, M A Denborough

    British Journal of Anaesthesia
    |April 1, 1986

    Summary

Suxamethonium chloride does not cause in vitro muscle contraction in susceptible swine. However, it triggers malignant hyperpyrexia in vivo, possibly due to increased myoplasmic calcium from muscle fasciculations.

    Area of Science:

    • Anesthesiology
    • Pharmacology
    • Veterinary Medicine

    Background:

    • Malignant hyperpyrexia (MH) is a severe pharmacogenetic disorder of skeletal muscle.
    • Swine susceptible to MH exhibit distinct responses to anesthetic agents.

    Purpose of the Study:

    • To investigate the in vitro and in vivo effects of suxamethonium chloride on swine susceptible to malignant hyperpyrexia.
    • To explore the mechanism by which suxamethonium chloride may induce MH.

    Main Methods:

    • In vitro contracture testing of swine skeletal muscle exposed to suxamethonium chloride.
    • In vivo administration of suxamethonium chloride to assess MH response in susceptible swine.

    Main Results:

    • Pure suxamethonium chloride did not induce in vitro contracture in skeletal muscle from MH-susceptible swine.
    • Suxamethonium chloride administration in vivo triggered malignant hyperpyrexia in MH-susceptible swine.

    Conclusions:

    • The in vitro findings suggest that suxamethonium chloride's direct effect on muscle is not the primary trigger for MH in swine.
    • In vivo MH induction is likely mediated by indirect effects, such as fasciculations leading to increased myoplasmic calcium levels.
