Background:

  • Hypothermic Machine Perfusion (HMP) is known to reduce delayed graft function (DGF) in deceased donor kidney transplants.
  • The protective mechanisms of HMP are not fully understood.
  • This study investigates the role of pro-inflammatory molecule clearance during HMP.

Purpose of the Study:

  • To test the hypothesis that HMP improves kidney transplant outcomes by clearing pro-inflammatory molecules from the allograft.
  • To prospectively evaluate the time-dependent clearance of cytokines, chemokines, and injury markers during HMP.

Main Methods:

  • Analysis of 26 donor kidney allografts preserved with pulsatile HMP.
  • Measurement of cytokines, chemokines, and injury markers in perfusate over time.
  • Assessment of T-cell activation by perfusate samples ex vivo.

Main Results:

  • Increased levels of IL-1RA, IL-6, IL-18, INFγ, and MCP-1 were observed in the perfusate over time.
  • These cytokine and chemokine increases correlated with kidney injury markers.
  • The 'danger' molecule HMGB1 appeared in the perfusate, and samples activated T-cells ex vivo.

Conclusions:

  • HMP facilitates a time-dependent clearance of pro-inflammatory cytokines, chemokines, and danger molecules from donor kidneys.
  • This clearance mechanism may contribute to the observed beneficial effects of HMP on kidney transplant outcomes.
