The Quality and Outcomes Framework (QOF) pay-for-performance program in UK general practice shows associations between practice characteristics and performance. Some factors, like socioeconomic deprivation and older patient populations, are linked to lower QOF scores, raising equity concerns.
Area of Science:
- Health Services Research
- General Practice Management
- Healthcare Policy Analysis
Background:
- The Quality and Outcomes Framework (QOF) is a significant pay-for-performance initiative in UK general practice, influencing practice income since 2004.
- Concerns exist regarding the QOF's impact on equity within general practice.
Purpose of the Study:
- To identify practice characteristics associated with performance in the Quality and Outcomes Framework (QOF).
Main Methods:
- A systematic review of studies examining the relationship between general practice characteristics and QOF performance in the UK.
- Searches included MEDLINE, Embase, CINAHL+, Web of Science, and grey literature.
Main Results:
- Twenty-two studies (2006-2022) analyzed population and organizational characteristics against QOF measures.
- Negative associations were found between QOF performance and socioeconomic deprivation, older patient populations, larger list sizes, older GP mean age, and Alternative Provider Medical Services contracts.
- Group practices, higher full-time equivalent (FTE) GP numbers, and training practices were linked to better QOF performance.
Conclusions:
- Practice characteristics, both controllable and uncontrollable, influence QOF performance.
- Pay-for-performance schemes may disadvantage practices serving high-risk populations (older, deprived).
- Further research with updated data is needed to confirm and understand persistent relationships.
