    Background:

    • Symptomatic ventricular tachycardia (VT) poses a significant clinical challenge.
    • Conventional antiarrhythmic drugs (Class I and III) are not always effective for VT management.

    Observation:

    • Two patients with symptomatic VT were treated with verapamil.
    • The first patient showed VT suppression with oral verapamil, suggesting triggered activity.
    • The second patient's VT was not inducible after verapamil administration during electrophysiologic study.

    Findings:

    • Oral verapamil demonstrated efficacy in suppressing VT in both cases.
    • The antiarrhythmic mechanism of verapamil in VT requires further elucidation.
    • Recurrence of VT symptoms was observed upon verapamil cessation in the first patient.

    Implications:

    • Verapamil may represent a therapeutic option for specific VT cases refractory to other treatments.
    • Robust methodologies are essential to validate verapamil's chronic efficacy in VT.
    • Understanding verapamil's mechanism in VT could refine treatment strategies.
