    Background:

    • Hypothermia decreases metabolic rate, potentially beneficial during low oxygen delivery (Do2).
    • Hypothermia increases hemoglobin's oxygen affinity, potentially hindering tissue oxygen release.

    Purpose of the Study:

    • To determine the net effect of hypothermia on peripheral tissue oxygen extraction.
    • To compare the hypoxemia response in hypothermic versus normothermic dogs.

    Main Methods:

    • Anesthetized, ventilated, and paralyzed dogs underwent progressive oxygen reduction until death.
    • Core body temperatures were maintained at normothermic (37.7°C) or hypothermic (30.5°C) levels.
    • Oxygen consumption (VO2) and oxyhemoglobin dissociation curves (ODC) were analyzed.

    Main Results:

    • Hypothermic dogs had lower VO2 (3.2 ml/min/kg) than controls (4.7 ml/min/kg).
    • Hypothermia shifted the ODC, lowering the P50 (19.8 Torr) compared to controls (32.4 Torr).
    • Oxygen delivery at which VO2 becomes supply-dependent (DO2crit) was lower in hypothermic dogs (6.2 ml/min/kg) versus controls (8.5 ml/min/kg).

    Conclusions:

    • Hypothermia's reduction in metabolic rate outweighs the adverse effect of increased hemoglobin-oxygen affinity.
    • Hypothermic dogs maintained baseline VO2 at lower arterial oxygen partial pressures.
    • Hypothermia enhances the ability of peripheral tissues to extract oxygen during hypoxemia.
