Background:

  • Trabecular metal is increasingly utilized in primary total hip arthroplasty (THA).
  • Comparison of medium-term outcomes (< 15 years) between fiber mesh titanium and trabecular metal acetabular components is essential.

Purpose of the Study:

  • To compare the medium-term outcomes of primary total hip arthroplasty (THA) using fiber mesh titanium versus trabecular metal acetabular components.
  • To evaluate survivorship and revision rates for these two acetabular implant types.

Main Methods:

  • A cohort of 6,563 patients undergoing primary THA was analyzed.
  • Data were collected from a prospectively maintained arthroplasty database and linked with the National Joint Registry.
  • Patients received either fiber mesh titanium or trabecular metal-backed acetabular components.

Main Results:

  • The 10-year survivorship was 97.3% for fiber mesh titanium and 98.9% for trabecular metal components (P = 0.009).
  • Multivariate analysis did not identify any significant variables associated with reduced revision rates.

Conclusions:

  • Both implant types exhibit high survivorship in THA.
  • Trabecular metal demonstrated statistically significant, albeit marginal, superiority in survival rates.
  • Trabecular metal may be advantageous in complex primary and revision cases requiring enhanced primary stability, despite higher costs.
