    Summary

The deltopectoral flap is preferred for cervical esophageal reconstruction after hypopharyngeal surgery. This method provides a reliable skin source, proving superior to penile skin grafts in clinical follow-up.

    Area of Science:

    • Surgical Oncology
    • Reconstructive Surgery
    • Otolaryngology

    Background:

    • Hypopharyngeal cancer often requires extensive surgical resection.
    • Cervical esophageal reconstruction is a critical step following hypopharyngeal excision.
    • Reconstructive techniques aim to restore function and minimize morbidity.

    Purpose of the Study:

    • To report experience with cervical esophageal reconstruction.
    • To compare the efficacy of two reconstructive methods: penile full-thickness skin graft and deltopectoral flap.
    • To determine the preferred method for post-hypopharyngectomy reconstruction.

    Main Methods:

    • Retrospective review of 11 cases undergoing hypopharyngeal excision and neck dissection.
    • Reconstruction utilizing penile full-thickness skin grafts (Kaplan and Markowicz) in 2 cases.
    • Reconstruction utilizing deltopectoral flaps (Bakamjian) in 9 cases.

    Main Results:

    • Clinical follow-up data were critically evaluated for both methods.
    • The deltopectoral flap demonstrated superior outcomes in this cohort.
    • Penile skin grafts were used in fewer cases compared to deltopectoral flaps.

    Conclusions:

    • The deltopectoral flap is the preferred method for cervical esophageal reconstruction after hypopharyngeal excision.
    • The deltopectoral flap offers a robust and well-vascularized tissue source for reconstruction.
    • This technique is considered the solution of choice for this complex surgical scenario.
