Oral Hypoglycemic Agents: α-Glucosidase Inhibitors01:19

Oral Hypoglycemic Agents: α-Glucosidase Inhibitors

159
α-glucosidase inhibitors, including acarbose (Precose), miglitol (Glyset), and voglibose (Voglib) (primarily available in Asia), are drugs that control blood sugar levels by delaying the digestion of starch and disaccharides. They achieve this by inhibiting α-glucosidase enzymes in the intestine, which slow the absorption of carbohydrates in the intestine, which in turn leads to a prolonged release of the glucoregulatory hormone GLP-1 from intestinal L-cells.

Dipeptidyl Peptidase 4 Inhibitors01:23

Dipeptidyl Peptidase 4 Inhibitors

171
Dipeptidyl peptidase 4 (DPP-4) is a serine protease widely distributed in the body. It's involved in the inactivation of GLP-1 and GIP hormones, which are crucial for insulin regulation. DPP-4 inhibitors, such as sitagliptin (Januvia), saxagliptin (Onglyza), linagliptin (Tradjenta), alogliptin (Nesina), and vildagliptin (Galvus), help increase the proportion of active GLP-1, enhancing insulin secretion. These inhibitors work by competitively binding to DPP-4. This binding causes a...

Area of Science:

  • Neurology
  • Pharmacology
  • Drug Repurposing

Background:

  • Ischemic stroke is a major global cause of mortality and long-term disability.
  • Current treatments like tissue plasminogen activator have limited therapeutic windows.
  • Post-stroke rehabilitation offers benefits but lacks promptness.

Purpose of the Study:

  • To review the potential of repurposing metformin for ischemic stroke treatment.
  • To compile and analyze data from preclinical and clinical trials.
  • To address discrepancies and propose solutions for metformin's use in stroke.

Main Methods:

  • Systematic review of preclinical studies.
  • Analysis of clinical trial data.
  • Evaluation of metformin's preventive and therapeutic effects in stroke.

Main Results:

  • Metformin has been extensively researched for stroke over the past decade.
  • Evidence suggests potential benefits in both preventing and treating ischemic stroke.
  • Discrepancies in current data require clarification.

Conclusions:

  • Drug repurposing, specifically with metformin, presents a promising avenue for expanding ischemic stroke therapeutic options.
  • Further research is needed to clarify discrepancies and optimize metformin's application.
  • Metformin could offer a valuable addition to stroke management strategies.
