    • A 53-year-old patient with active rheumatoid arthritis was treated with D-penicillamine.
    • The patient developed idiopathic nephrotic syndrome.
    • Renal biopsy revealed IgM nephropathy.

    Findings:

    • This is the first reported case of D-penicillamine-induced IgM nephropathy.
    • The patient's nephrotic syndrome was associated with specific IgM glomerular alterations.
    • The findings suggest a potential link between D-penicillamine and this specific form of glomerulonephritis.
