    Background:

    • External ear canal cholesteatoma (EECC) is a rare otologic condition.
    • Historically confused with keratosis obturans, current understanding differentiates them based on clinical and pathological findings.

    Observation:

    • Eight patients with EECC were treated.
    • Consistent findings include erosion of the inferior canal wall and keratin debris accumulation.

    Findings:

    • Local debridement and curettage are effective for limited EECC lesions.
    • Canalplasty or tympanomastoidectomy are indicated for more extensive EECC.

    Implications:

    • Differentiating EECC from keratosis obturans is crucial for appropriate management.
    • Tailored surgical approaches based on lesion extent optimize treatment outcomes for EECC.
