A retroverted uterus in early pregnancy is linked to a higher risk of first-trimester bleeding but not pregnancy loss. More patients with a retroverted uterus conceived via in vitro fertilization (IVF).
Area of Science:
- Obstetrics and Gynecology
- Reproductive Medicine
- Fetal Ultrasound
Background:
- The association between a retroverted uterus identified during nuchal translucency (NT) ultrasound and pregnancy outcomes is not well-established.
- Understanding these associations is crucial for accurate patient counseling and management.
Purpose of the Study:
- To investigate the relationship between a retroverted (RV) uterus detected on NT ultrasound and second-trimester pregnancy loss.
- To explore other adverse pregnancy outcomes associated with a retroverted uterus.
Main Methods:
- Retrospective cohort study comparing patients with a retroverted uterus (RV) to those with an anteverted (AV) uterus.
- Exclusion criteria included multiple gestation, Mullerian anomalies, and fetal aneuploidy.
- Statistical analysis used Fisher's exact test, Mann-Whitney test, and multivariable logistic regression.
Main Results:
- No significant difference in second-trimester pregnancy loss was observed between RV and AV groups (0% vs. 1.2%, p=0.55).
- Patients with a retroverted uterus were four times more likely to experience first-trimester vaginal bleeding (16.9% vs. 4.2%, OR=4.1, p=0.0056) after adjusting for race and IVF.
- A significantly higher proportion of RV patients conceived via in vitro fertilization (IVF) (11% vs. 3%, p=0.018).
Conclusions:
- A persistent retroverted uterus in the first trimester is associated with an increased risk of first-trimester vaginal bleeding.
- Despite the increased bleeding risk, pregnancy loss rates were similar, offering valuable information for patient counseling.
- The higher incidence of IVF conception in the RV group warrants further investigation.
