Abstract:
The 78 Nord Porte de Normandie territorial professional health communities (CPTS), created in 2020 in the Yvelines department, aims to improve care coordination and access to healthcare services for a population of 170,000 inhabitants spread over 57 communes. Driven by the Odyssée association, the project has been designed to meet the challenges of a region marked by social inequalities, unfavorable medical demographics and difficulties in accessing care. It stands out for its inclusive governance, bringing together healthcare professionals, users and local players, enabling decisions to be taken in line with the realities on the ground. Thanks to its collaborative approach and the commitment of its members, the CPTS has established itself as an essential pillar in the reorganization of care in its area.
