Summary

Primary spinal chondrosarcoma is a rare bone cancer. This case highlights diagnosis and treatment of a T5-T6 thoracic spinal chondrosarcoma in a young man.

Background:

Chondrosarcomas are malignant tumors producing cartilaginous matrix, categorized as primary or secondary.
Primary chondrosarcomas rank as the third most common primary malignant bone tumors, accounting for 20-27% of all such cases.
Primary spinal chondrosarcoma is exceptionally rare, with the thoracic spine being the most frequent site.

Observation:

A 36-year-old male presented with a 3-year history of worsening back pain and swelling.
Radiography revealed a lytic sclerotic expansile lesion at the T5-T6 level.
MRI demonstrated intradural and intramedullary infiltration, causing spinal canal stenosis.

Findings:

CT scans showed an expansile mass with the characteristic "rings and arcs" appearance of chondrosarcoma.
Histological examination confirmed a low-grade chondrosarcoma (Grade I).
The tumor infiltrated surrounding spinal ligaments and caused severe spinal canal stenosis.

Implications:

En bloc resection combined with adjuvant radiotherapy offers the most effective treatment for spinal chondrosarcoma.
Surgical intervention and radiotherapy can lead to significant neurological improvement.
Long-term monitoring is essential for detecting recurrence and optimizing patient outcomes.
