Summary

Dupilumab use for atopic dermatitis (AD) is linked to a higher risk of cutaneous T-cell lymphoma (CTCL). This risk is most pronounced in patients over 60 and within the first year of dupilumab therapy.

Background:

  • Reports suggest a link between dupilumab and cutaneous T-cell lymphoma (CTCL).

  • Atopic dermatitis (AD) is a common inflammatory skin condition.

Purpose of the Study:

  • To determine if dupilumab for AD increases CTCL risk.
  • To identify patient subgroups at higher risk for CTCL development.
  • To evaluate the temporal relationship between dupilumab initiation and CTCL diagnosis.

Main Methods:

  • Retrospective cohort study utilizing the TriNetX deidentified medical record database.
  • Inclusion of over 1.18 million patients with AD.
  • Matching of dupilumab-treated patients with a control group receiving alternative AD therapies.

Main Results:

  • Dupilumab use was associated with a 4.59-fold increased relative risk of CTCL (RR = 4.59, 95% CI 2.459-8.567, P < 0.0001).
  • Over half of CTCL cases occurred in patients over 60 years old.
  • The majority of CTCL diagnoses post-dupilumab occurred within the first year of treatment.

Conclusions:

  • Dupilumab treatment for AD is associated with an elevated risk of developing CTCL.
  • The risk is significantly higher in patients over 60 and within the first year of therapy.
  • Caution is advised when prescribing dupilumab to specific patient populations.
