Background:

  • Appendiceal neoplasms are uncommon and typically present with subtle, non-specific symptoms.
  • Their rarity and vague presentation often lead to delayed diagnosis, particularly in women presenting with right iliac fossa pain.

Observation:

  • A 56-year-old woman presented with a right pelvic mass, initially suspected to be of ovarian origin.
  • Exploratory laparotomy revealed normal uterus and adnexa; the mass originated from the appendix, extending into the right adnexal region.
  • The mass was in contact with an atrophic right ovary.

Findings:

  • The resected appendix vermiformis was confirmed pathologically as an acellular mucinous tumor.
  • Preoperative diagnosis of appendiceal mucinous tumors remains challenging.

Implications:

  • This case underscores the diagnostic difficulty of appendiceal mucoceles, often discovered incidentally during imaging or surgery.
  • Gynecologists should consider appendiceal pathology in women with right iliac fossa masses, even when ovarian pathology is suspected.
  • Improved preoperative diagnostic accuracy for appendiceal neoplasms is needed.
