    Background:

    • Two pediatric cases presented with distinct clinical features.
    • Patient 1: acute renal failure, hypertension, hyponatremia.
    • Patient 2: fever, hypernatremia, hypokalemia, followed by hypertension.

    Findings:

    • Both patients exhibited dark, o-tolidine-positive urine and granular casts.
    • Markedly elevated serum creatine kinase levels ( > 60,000 U/L) confirmed muscle damage.
    • These findings are indicative of significant skeletal muscle injury.

    Implications:

    • Rhabdomyolysis in children, though uncommon, requires prompt recognition.
    • Simple laboratory tests are effective in diagnosing this potentially severe condition.
    • Early diagnosis and management are vital to prevent complications like acute kidney injury.
