    Background:

    • The direct mechanical effects of lung expansion on circulation are well-known.
    • However, the reflex cardiovascular responses to lung inflation and their underlying neural mechanisms require further elucidation.

    Purpose of the Study:

    • To determine the cardiovascular reflex responses initiated by lung inflation.
    • To identify the autonomic neural mechanisms mediating these reflex cardiovascular effects.

    Main Methods:

    • Isolated left lung preparations in dogs were used to isolate reflex effects from mechanical ones.
    • Static lung inflations were performed, and cardiovascular parameters (left ventricular pressure, heart rate, vascular resistance, cardiac output) were measured.
    • Vagotomy was performed to assess the role of vagal pathways.
    • Cardiac pacing was employed to eliminate confounding mechanical effects (Bowditch effect).

    Main Results:

    • Left lung inflation (LLI) significantly decreased left ventricular systolic pressure, heart rate, and hindlimb vascular resistance.
    • LLI reduced left ventricular contractility and cardiac output (stroke volume) independently of mechanical effects.
    • Vagotomy largely abolished the cardiovascular responses to LLI.
    • Hindlimb vasodilation was mediated by alpha-adrenergic activity; reduced contractility by decreased beta-adrenergic activity; reflex bradycardia by increased cholinergic and decreased beta-adrenergic activity.

    Conclusions:

    • Lung expansion can elicit significant cardiovascular reflex responses.
    • Autonomic neural pathways, particularly via the vagus nerve, are crucial in mediating these reflexes.
    • Lung inflation reflexes modulate heart rate, ventricular contractility, systemic vasomotor tone, cardiac output, and stroke volume.
