Immunotherapy is a treatment that boosts or manipulates the immune system to fight diseases, including cancer. For instance, by stimulating an immune response through vaccinations against viruses that cause cancers, like hepatitis B virus and human papillomavirus, these diseases can be prevented. Nonetheless, some cancer cells can avoid the immune system due to their rapid mutation and division. The immune response to many cancers involves three phases: elimination, equilibrium, and escape.

Immune checkpoint inhibitors (ICI) show variable response in advanced sarcoma. Certain subtypes like Kaposi sarcoma and alveolar soft part sarcoma had higher response rates, while PD-L1 expression correlated with improved outcomes.

Background:

  • Immune checkpoint inhibitors (ICIs) represent a significant advancement in cancer therapy.
  • Sarcomas are a heterogeneous group of rare cancers with diverse clinical behaviors.
  • Predicting response to ICIs in advanced sarcoma remains a challenge due to tumor heterogeneity.

Purpose of the Study:

  • To identify factors associated with response to immune checkpoint inhibitors (ICI) in patients with advanced sarcoma.
  • To analyze the impact of histologic subtypes, tumor mutational burden, and PD-L1 expression on ICI efficacy.
  • To characterize overall survival, progression-free survival (PFS), and objective response rates (ORR) in advanced sarcoma treated with ICI.

Main Methods:

  • Retrospective analysis of 216 advanced sarcoma patients treated with ICI from 2016-2023.
  • Evaluation of overall survival, PFS, and ORR using RECIST criteria.
  • Assessment of response across different sarcoma histologic subtypes, ICI regimens, tumor mutational burden, and PD-L1 expression levels.

Main Results:

  • The overall objective response rate (ORR) was 16.7%.
  • Histologic subtypes with highest ORR included Kaposi sarcoma (66.7%) and alveolar soft part sarcoma (50%).
  • PD-L1 expression (≥1%) was associated with a higher ORR (27.8%, P=0.02).

Conclusions:

  • Sarcoma histologic subtypes significantly influence response and progression-free survival (PFS) to ICI therapy.
  • Kaposi sarcoma, alveolar soft part sarcoma, angiosarcoma, myxofibrosarcoma, and undifferentiated pleomorphic sarcoma showed the best outcomes.
  • PD-L1 expression is a potential biomarker for predicting ICI response in advanced sarcoma.
