    A patient with a frontal lobe hematoma experienced sustained eye deviation. Recovery revealed lasting deficits in eye movements, including saccades and smooth pursuit, suggesting an imbalance in smooth eye movement control.

    Background:

    Observation:

    • A patient presented with sustained contralateral eye deviation lasting one week.
    • Following recovery from deviation, the patient exhibited hypo-metric ipsilateral saccades and prolonged saccadic latencies in both horizontal directions.

    Findings:

    • Smooth pursuit eye movements were impaired (paretic) when tracking towards the side of the hematoma.
    • Smooth pursuit gain was elevated during tracking towards the contralateral side, indicating an imbalance.

    Implications:

    • The observed smooth eye movement imbalance provides a potential explanation for the initial contralateral gaze deviation.
