Emergency laparoscopic common bile duct exploration (ELCBDE) is a safe and effective treatment for choledocholithiasis, demonstrating low complication rates. This approach is suitable for elderly patients and those with high comorbidity, offering a viable alternative for urgent biliary decompression.
Area of Science:
- Hepatobiliary surgery
- Minimally invasive surgery
- Gastroenterology
Background:
- Choledocholithiasis often requires emergency treatment due to severe complications like cholangitis.
- Endoscopic retrograde cholangiopancreatography (ERCP) is standard, but emergent access is limited.
- Laparoscopic common bile duct exploration (LCBDE) shows promise in elective settings.
Purpose of the Study:
- To evaluate the outcomes of emergency laparoscopic common bile duct exploration (ELCBDE) for choledocholithiasis.
- To assess the safety and efficacy of ELCBDE in an emergency setting.
- To determine if patient factors like age and comorbidity affect outcomes.
Main Methods:
- A prospective, single-center study of 31 patients undergoing ELCBDE from January 2020 to December 2023.
- Data collected included demographics, clinical presentation, surgical variables, and postoperative outcomes (complications, stone clearance, bile leaks).
- Analysis of complication rates, stone recurrence, and association with Charlson Comorbidity Index (CCI) and age.
Main Results:
- ELCBDE was performed successfully in all 31 patients without conversion to open surgery.
- Low rates of severe complications (6.5%) and no reinterventions or technique-related deaths were observed.
- Median hospital stay was 4 days, with no detected stone recurrence. Neither CCI nor advanced age correlated with severe complications.
Conclusions:
- Emergency laparoscopic common bile duct exploration (ELCBDE) is an effective and safe procedure for choledocholithiasis.
- Patients with high CCI or advanced age can be considered for emergent laparoscopic treatment.
- ELCBDE should be considered a primary option for emergency management of choledocholithiasis.
