Background:

  • Sinonasal inverted papilloma (SNIP) is a benign tumor with a significant recurrence rate.
  • The role of human papillomavirus (HPV) infection in SNIP recurrence remains controversial.
  • Previous studies have yielded conflicting results regarding the association between HPV and SNIP recurrence.

Purpose of the Study:

  • To systematically review and meta-analyze existing literature to clarify the association between HPV infection and SNIP recurrence.
  • To determine the pooled odds ratio (OR) for recurrence in HPV-positive versus HPV-negative SNIP.
  • To investigate if HPV subtype (low-risk vs. high-risk) influences SNIP recurrence risk.

Main Methods:

  • A systematic search of PubMed, Embase, and Scopus databases was conducted.
  • Included case-control studies reporting SNIP recurrence and HPV status determined by PCR and ISH.
  • Meta-analysis was performed to calculate pooled ORs and 95% confidence intervals (CIs).

Main Results:

  • Twenty-five studies comprising 1116 benign SNIP tumors were analyzed.
  • HPV-positive SNIP demonstrated a pooled OR of 2.05 (95% CI: 1.31-3.19) for recurrence compared to HPV-negative tumors.
  • The increased risk of recurrence associated with HPV was not statistically significant between low-risk and high-risk HPV subtypes.

Conclusions:

  • HPV infection is associated with an increased risk of SNIP recurrence.
  • This increased risk appears independent of HPV subtype.
  • Variability in recent studies necessitates further investigation into the HPV-SNIP recurrence relationship.
