Background:

  • Chronic diseases are rising due to sedentary lifestyles and poor health choices.
  • Lifestyle medicine offers effective prevention and treatment but is underutilized.
  • The Complete Lifestyle Medicine Intervention Program-Ontario (CLIP-ON) was created to address this gap in Northern Ontario.

Purpose of the Study:

  • To evaluate the CLIP-ON program's implementation and effectiveness for chronic disease patients in Parry Sound, Ontario.
  • To assess changes in lifestyle behaviors and health outcomes.
  • To understand enrollment, retention, and interdisciplinary team engagement.

  • Observational cohort study using the RE-AIM framework and a hybrid type II mixed methods design.
