Summary

Chinese American patients with chronic rhinosinusitis (CRS) are less likely to choose endoscopic sinus surgery (ESS) than non-Chinese Americans. However, ESS effectively improves quality of life for Chinese American patients with CRS.

Background:

Limited research exists on how race and ethnicity influence treatment decisions for chronic rhinosinusitis (CRS).
Understanding these disparities is crucial for equitable patient care.

Purpose of the Study:

To investigate factors influencing treatment modality choice in medically refractory CRS.
To compare treatment decisions between Chinese American and non-Chinese American patients.

Main Methods:

Prospective enrollment of CRS patients with persistent symptoms.
Patients chose between continued medical treatment or endoscopic sinus surgery (ESS).
Multivariable logistic regression analyzed the association between ethnicity and treatment choice, adjusting for covariates.

Main Results:

Of 134 patients, 59% opted for ESS. Non-Chinese Americans were significantly more likely to choose ESS (OR=7.92).
No significant baseline differences were found between groups regarding demographics or disease severity.
Chinese American patients undergoing ESS showed significant SNOT-22 score improvements postoperatively.

Conclusions:

Chinese American patients with refractory CRS demonstrate a lower propensity for ESS compared to non-Chinese American patients.
ESS is an effective treatment for improving quality of life in Chinese American CRS patients.
Further research is needed to identify barriers to surgical treatment among this population.
