In human women, oogenesis produces one mature egg cell or ovum for every precursor cell that enters meiosis. This process differs in two unique ways from the equivalent procedure of spermatogenesis in males. Background:

Ovarian adenomyoma is a rare gynecological tumor with a high misdiagnosis rate.
Menstrual abdominal pain is a common symptom, often leading to diagnostic challenges.
Current diagnostic methods frequently result in misdiagnosis, necessitating intraoperative confirmation.

Observation:

A 50-year-old woman presented with menstrual-related lower abdominal pain.
Imaging revealed multiple uterine and ovarian masses, with elevated CA125 levels.
Magnetic resonance imaging (MRI) confirmed uterine fibroids and suggested benign cystic degeneration.

Findings:

Postoperative pathology confirmed ovarian adenomyoma, a rare benign gynecologic tumor.
The case highlights the diagnostic difficulties associated with ovarian adenomyoma.
Surgical resection was effective, with pathology confirming the diagnosis.

Implications:

Consider extrauterine adenomyosis in patients with specific symptoms and MRI findings.
Minimally invasive treatments may be beneficial, but malignancy potential requires caution.
Improved diagnostic strategies are crucial for reducing misdiagnosis and preserving fertility.
